Signature HealthCARE seeks a compassionate Registered Nurse to deliver person-centered care in our long-term and post-acute communities. The RN will assess residents, develop and implement care plans, administer medications and treatments, and coordinate with an interdisciplinary team to optimize outcomes. You will educate residents and families, document accurately, and uphold safety, quality, and regulatory standards. Responsibilities Provide comprehensive nursing assessments and ongoing evaluations of residents' conditions Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team Administer medications and treatments safely and in accordance with physician orders and facility policies Monitor, document, and report changes in residents' status to providers and families Ensure compliance with clinical protocols, infection control, and regulatory standards Educate residents and families on conditions, medications, and care plans Supervise, mentor, and support CNAs and other care team members Respond promptly to emergencies and perform appropriate interventions Promote a resident-centered, dignified, and compassionate care environment Participate in quality improvement, care conferences, and ongoing training Required Skills Patient assessment Care planning and care coordination Medication administration and pharmacology Wound care and skin integrity management IV therapy and venipuncture Electronic Health Record (EHR) documentation Infection prevention and control Patient and family education Clinical triage and critical thinking Supervision and delegation of clinical staff
