Transitions Care is seeking a compassionate Registered Nurse to provide hospice visit care in patients' homes and facilities. In this role, you'll deliver skilled nursing assessments, manage symptoms, administer medications, and implement individualized care plans focused on comfort and quality of life. You will educate and support patients and families, coordinate closely with an interdisciplinary hospice team, and ensure timely documentation. Ideal candidates demonstrate strong clinical judgment, hospice or home health experience, excellent communication, and a deep commitment to dignity, respect, and holistic end-of-life care. Responsibilities Provide skilled hospice nursing visits in homes and facilities Complete comprehensive assessments and ongoing symptom management Administer medications and treatments per care plan and orders Develop, implement, and update individualized hospice care plans Educate patients and families on disease process, comfort measures, and medications Coordinate care with physicians, social workers, chaplains, and other team members Document assessments, interventions, and outcomes accurately and timely in EMRParticipate in on-call rotation and respond to urgent patient needs Advocate for patient comfort, dignity, and end-of-life wishes Ensure compliance with hospice regulations, policies, and best practices Required Skills Hospice nursing Home health nursing Patient assessment Pain and symptom management Medication administration Care planning Electronic medical records (EMR) documentation IV and wound care skills Patient and family education Interdisciplinary team collaboration
