Job Description

Join our team as an RN in the "Better Birth Outcomes" and "Family Connects IL" program and help Stephenson County high risk pregnant mothers and families with high-risk infants get the best start in life! As part of highly integrated team, you will:
  • Provide a health and physical assessment of mother and baby in the home to determine well-being.
  • Assess family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence. Exercise clinical skills in appraising situations and making decisions.
  • Establish a trusting relationship with families during in-home visits and engage the family to establish a family support plan with goals, objectives, and activities that address family strengths and needs.
  • Work with a team to ensure appropriate referrals and connections to community resources as needed. Work in a team to assist parents in making and attending appointments for doctors, social service agencies, etc. Act as a liaison between families and other community agencies, serving as a family advocate as required.
  • Apply working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assist in creating a health plan for entire family.
  • Review known risk factors and gather resources in anticipation of the visit with the family.
  • Obtain the family's desired goals for health and wellness.
  • Assess current and relevant past concerns including reproductive/pregnancy health history, medical/surgical history, psychosocial history, and list of prescription and over-the-counter medications or supplements.
  • Complete head to toe physical assessment of pregnant or postpartum women and infants including vital signs, height, weight and head circumference (infant only).
Provide referrals and assist with linkages if indicated below: If pregnant, include assessment for pregnancy warning signs and patient-reported fetal movement. If postpartum, include postpartum warning signs and surgical wounds tears and reproductive well-being. If infant, review developmental/growth and nutritional status and provide anticipatory guidance.
  • For any abnormal findings or complications, provide interventions as needed.
  • Using clinical judgement, discuss plan for follow-up in conjunction with the Nurse Navigator if needed.
  • Documents all necessary information on the client record according to established protocols.
  • Serves as client advocate in obtaining services.
  • Identifies opportunities during daily case coordination to inform people of resources available through various health department programs.
  • Maintains accurate accountability of case management services for purposes of reporting as required by CMIS.
  • Attends seminars, conferences and workshops as available on pertinent maternal child health subjects to stay informed of health care and management of clients.
  • Performs other duties as required or assigned.

Pay:
$31.92 - $31.93 per hour
