Job Description
Responsibilities:
- Provide skilled nursing care to patients in their homes
- Assess patient's condition and develop a comprehensive care plan
- Administer medications and treatments as prescribed by the physician
- Monitor and evaluate patient's progress and adjust care plan as needed
- Educate patients and their families on proper healthcare techniques and disease management
- Coordinate with other healthcare professionals to ensure continuity of care
- Document all patient interactions and interventions accurately and timely
Requirements:
- Valid RN license in the state of Illinois
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills
- Knowledge of medical terminology and procedures
- Familiarity with electronic medical records (EMR) systems, such as Epic
- Ability to lift and transport patients as needed We are looking for a compassionate and dedicated Registered Nurse to join our Home Health team.
