MDS Care Plan Coordinator (RN) Join Our Team at Meadowbrook of La Grange Meadowbrook of La Grange is seeking an experienced and detail-oriented MDS Care Plan Coordinator (RN) to join our team. This is a great opportunity for a nurse who enjoys focusing on assessments, care planning, and compliance —with no floor nursing responsibilities required . About the Role As the MDS Care Plan Coordinator, you will play a key role in ensuring compliance, accuracy, and quality in resident care planning. You will collaborate closely with the interdisciplinary team to support optimal outcomes for our residents. Key Responsibilities Complete accurate and timely MDS assessments for all residents Develop, implement, and monitor individualized care plans Manage MDS and care plan schedules, including new admissions and significant changes Ensure timely submission of MDS assessments to appropriate agencies Maintain compliance with state, federal, and local regulations Collaborate with interdisciplinary teams to enhance resident care Apply strong critical thinking and communication skills in daily tasks Qualifications Active Illinois RN License in good standing Minimum 1+ year experience in LTC/SNF as an MDS Coordinator Strong organizational and time management skills Knowledge of regulatory standards and compliance requirements Excellent communication and clinical assessment skills Why Join Meadowbrook?
