Key Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage members during in-home visits (approximately 45 minutes each) Evaluate patients' medical histories, prior conditions, current comorbidities, and medication regimens Educate patients on medical conditions, prescribed medications, and general wellness Identify gaps in care and provide actionable recommendations for closing them Deliver post-discharge assessments as needed, with no requirement to create treatment plans or prescribe medications

Qualifications:

Active, unrestricted Nurse Practitioner license in Illinois Current CPR (BLS) certification Board Certification through either AANP or ANCC, required Completion of an MSN from an accredited nursing school At least 6 months of Nurse Practitioner experience preferred Compensation for this position is offered at $80$90 per evaluation, along with a mileage stipend and administrative time.
