Job Overview We are seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to provide high-quality skilled nursing care patients throughout the community. What You'll Do Perform comprehensive nursing assessments and develop individualized care plans. Provide skilled nursing services in accordance with physician orders and the plan of care. Monitor patient progress and communicate changes to physicians and the interdisciplinary team. Educate patients and caregivers on medications, disease management, safety, and self-care Complete accurate and timely clinical documentation. Collaborate care with physicians, therapists, aides, physicians, and other members of the interdisciplinary team. Complete accurate and timely documentation in accordance with Medicare and agency requirements. Help patients achieve their highest level of independence Maintain compliance with Medicare, state, and agency policies and procedures. What We're Looking For Required Current, unrestricted RN license, CPR certification and valid driver's license Reliable transportation and willingness to travel to patient homes Compassionate, professional, and patient-centered approach Excellent assessment, communication, and organizational skills Home health experience preferred, but qualified nurses with strong clinical backgrounds are encouraged to apply Knowledge of Medicare home health requirements and documentation

$85,000.00 - $95,000.00 per year
