This position primarily supports Lombard, Wheaton, Glen Ellyn, Villa Park, Elmhurst, Downers Grove, Westmont, Oak Brook, Hinsdale, Naperville, Woodridge and surrounding DuPage County communities. What You'll Do Manage an assigned hospice caseload Complete nursing assessments and scheduled visits Monitor symptoms and changes in condition Develop and update individualized plans of care Coordinate medications, equipment and patient needs Educate and support patients and families Communicate with physicians, facilities and the interdisciplinary team Complete accurate and timely documentation Independently organize your daily field schedule What You Bring Current Illinois RN license Strong nursing assessment and clinical judgment Excellent communication and follow-through Strong organization and documentation skills Ability to work independently while staying connected to the clinical team Reliable vehicle, valid driver's license and current auto insurance Hospice experience preferred What We Bring $83,000
