Key Responsibilities:

Conduct annual and post-discharge home health risk assessments, each averaging 45 minutes Review and evaluate medical histories, current health conditions, and medication usage Provide clear education to patients about their health concerns and prescribed medications Proactively identify gaps in care and offer practical recommendations for follow-up No requirements to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Illinois Current CPR certification (BLS) Board Certification through AANP or ANCC At least 6 months of recent experience working as an NP is preferred Master of Science in Nursing (MSN) degree from an accredited program This role offers compensation of $80$90 per evaluation, in addition to a mileage stipend and administrative timereflecting the value of your expertise and commitment.
