Make an Impact in Community Health Community Health Partnership of Illinois (CHP) is a nonprofit healthcare organization providing primary medical, dental, and behavioral health services to migrant farmworkers and other medically underserved populations throughout Illinois. We are seeking a compassionate and dependable Registered Nurse (RN) to join our Mendota healthcare team. This position is ideal for an RN who enjoys patient-centered care, working collaboratively with providers, and making a meaningful difference in the health of the community. What You'll Do Assess patients and monitor changes in health status Obtain and document vital signs, medical histories, and other clinical information Provide nursing care according to established clinical protocols and provider direction Administer medications, immunizations, and treatments as ordered Assist providers with examinations, procedures, and follow-up care Provide patient education regarding medications, treatment plans, preventive care, and chronic disease management Coordinate patient care and communicate with providers and other members of the healthcare team Respond appropriately to patient questions and clinical concerns Maintain accurate, timely, and confidential documentation in the electronic health record Support infection-control, quality, and patient-safety practices Help connect patients with appropriate follow-up care and community resources Maintain compliance with applicable clinical, organizational, and regulatory requirements What We're Looking For Current, active Registered Nurse (RN) license in Illinois Graduate of an accredited nursing program Previous outpatient, primary care, community health, or other clinical experience preferred Strong clinical assessment and critical-thinking skills Excellent communication and patient-service skills Ability to work both independently and collaboratively as part of an interdisciplinary healthcare team Strong organizational skills and attention to detail Commitment to providing respectful, culturally responsive care to patients from diverse backgrounds Bilingual English/Spanish skills are a plus. Compensation:
