Collects comprehensive data pertinent to the patient's health or the situation. Analyzes the assessment data to determine the diagnoses or issues. Identifies realistic and measurable outcomes which are complimentary to medical plan of care and centered on the patient's and family's/significant other's needs and appropriate practice guidelines. Collaborates with co-workers, physician, other disciplines, patient, and family to develop individualized plan of care to address patient diagnoses or issues that attains patient outcomes. Coordinates care delivery- Assumes responsibility for assigning and coordinating patient care with ancillary personnel consistent with patient needs, staff competencies and licensure.
Qualifications:
Education:
Must be a graduate of accredited School of Nursing. License(s)/Certification(s): Current Registered Professional Nurse licensure in the State of Illinois. Current licensure in good standing to practice as a Registered Nurse in Illinois Oncology Certification preferred. Current CPR Certification. Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs.
