Job Description

Role:
Registered Nurse, Case Manager (RNCM)
Overview:
This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families.
Key Responsibilities:
Manage patient care in hospice settings. Coordinate with healthcare teams. Provide leadership and support to nursing staff.
Schedule and Shift Details Schedule:
M-F ; 8a-5p (Start and end times vary from day to day
  • expected to be available from 8am
  • 5pm daily.) o Weekends
  • (1) 24 hour weekend day shift per month AND (2) weekday on calls per month  (1) 24 hour weekend day per month: 8A to 5P on Saturday or Sunday; will have scheduled visits ahead of time & seeing patients in the field
  • beginning 5pm until 8am, will be a true on call.
FYI:
We do also have dedicated After Hours RN calls off. (always 3 dedicated after hour RNs on every night)  Additional stipends and compensation for fulfilling on call  (2) weekdays of 5P to 8A  (2) holidays per year
  • 1 summer & 1 winter; 3 floating holidays Travel Local travel required covering multiple counties.
