Job Description
Signify Health is seeking a part-time In-Home Nurse Practitioner to perform comprehensive health evaluations for adults and seniors in their homes. You will conduct detailed assessments, review medications, manage chronic conditions, and identify gaps in care, using mobile technology and data-driven tools. Collaborating with interdisciplinary teams, you'll document findings in the EHR, coordinate follow-up care, and support value-based care initiatives.
