You'll conduct comprehensive, in-home health risk assessments, facilitating proactive health management for members at various stages of care, including annual evaluations and post-discharge visits.

Key Responsibilities:

Perform detailed home health risk assessments (~45 minutes each) for Medicare Advantage plan members Review and document patients medical histories, current health conditions, and medication usage Educate members about their health, conditions, and medication regimens Identify and communicate care gaps, recommending appropriate next steps No requirement for prescribing medications or developing ongoing treatment plans

Qualifications:

Active Nurse Practitioner license in Illinois Current CPR (BLS) certification Board certified via AANP or ANCC Minimum six months of recent Nurse Practitioner experience preferred MSN from an accredited nursing institution This role offers compensation of $80$90 per evaluation, along with a mileage stipend and support for administrative tasks.
