Schedule:
Monday-Friday | Daytime visits
Pay:
$53 - $72 per hour Mileage reimbursement provided.

LifeChoice Hospice and Palliative Care 800 E Northwest Hwy, Suite 506 Palatine, IL 60074 Role Overview We are seeking an experienced Hospice Nurse Practitioner to join our growing interdisciplinary team. This role focuses on patient evaluations, symptom management, and face-to-face recertification visits while working closely with the hospice medical director and clinical team to ensure compassionate, high-quality end-of-life care. Direct hospice experience is required for this position. What You Bring Active Illinois Nurse Practitioner license National Nurse Practitioner certification Direct hospice experience required Strong clinical judgment and patient assessment skills Ability to collaborate with physicians and interdisciplinary teams Excellent communication and documentation skills Compassionate approach to patient and family care Ability to travel locally for patient visits Your Real Day Conduct patient visits in homes, assisted living communities, and skilled nursing facilities Perform face-to-face hospice recertification visits Collaborate closely with hospice physicians and RN case managers Evaluate symptom management and recommend care adjustments Participate in Interdisciplinary Group (IDG) meetings Communicate with families, caregivers, and facility staff Review hospice eligibility and clinical documentation Support high-quality, compassionate end-of-life care Clinical Collaboration Our Nurse Practitioners work closely with the Hospice Medical Director and interdisciplinary clinical team to support patient care, symptom management, and hospice eligibility decisions. This role focuses on thoughtful clinical evaluation and collaboration, allowing providers to focus on quality patient care and meaningful family communication.
