What Your Day Looks Like As an RN Case Manager, you will manage and coordinate care for your assigned hospice patients in homes, assisted living communities, skilled nursing facilities, and other care settings. You will complete assessments and visits, manage symptoms, communicate changes in condition, coordinate medications and orders, educate families, and work closely with physicians, CNAs, social workers, chaplains, and the rest of the interdisciplinary team. You will have the independence to manage your day while still having a team behind you when you need support. More Than a Caseload Great hospice nursing is not about rushing from visit to visit.
