Hospice RN Case Manager Full-Time | North & Northwest Chicago / North Shore Know Your Patients. Own Your Care. Grow With Us. LifeChoice Hospice & Palliative Care is a nurse-led, 4.9-star rated hospice looking for an experienced RN Case Manager who wants to practice hospice the way it should feel: personal, thoughtful, organized, and centered around the patient. We are growing, and we are looking for nurses who want to grow with us. This is a role for an RN who likes building real relationships with patients and families, taking ownership of their care, and working with a team that values clinical judgment instead of treating nurses like numbers. What Your Day Looks Like As an RN Case Manager, you will manage and coordinate care for your assigned hospice patients in homes, assisted living communities, skilled nursing facilities, and other care settings. You will complete assessments and visits, manage symptoms, communicate changes in condition, coordinate medications and orders, educate families, and work closely with physicians, CNAs, social workers, chaplains, and the rest of the interdisciplinary team. You will have the independence to manage your day while still having a team behind you when you need support. More Than a Caseload Great hospice nursing is not about rushing from visit to visit.
