Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Evaluate patient medical histories, existing health conditions, and medication regimens Provide clear education to patients regarding medical conditions and proper medication usage Pinpoint gaps in care and suggest actionable recommendations to enhance patient outcomes Focus exclusively on assessmentsno requirement to prescribe or create direct treatment plans

Qualifications:

Active Nurse Practitioner license in the state of Illinois Current BLS/CPR certification Board Certification by AANP or

ANCC MSN
