Transitions Care is seeking a Director of Clinical Operations RN to lead hospice services focused on compassionate, high-quality end-of-life care. In this leadership role, you will oversee clinical operations, ensuring regulatory compliance, excellent symptom management, and coordinated support for patients and families. You'll mentor an interdisciplinary team, optimize workflows, track quality outcomes, and partner with leaders on growth initiatives in a mission-driven, patient-centered environment. Responsibilities Oversee day-to-day hospice clinical operations across service locations Ensure high-quality, patient- and family-centered end-of-life care Maintain compliance with hospice regulations, policies, and accreditation standards Lead, mentor, and develop interdisciplinary clinical teams Monitor clinical quality metrics and implement performance improvements Collaborate with physicians, nursing, social work, and spiritual care teams Manage staffing, scheduling, and resource allocation for clinical services Support complex case reviews and resolve clinical escalations Develop and refine clinical protocols, workflows, and best practices Partner with leadership on strategic growth and service enhancements Required Skills Hospice clinical management Nursing leadership Regulatory and compliance knowledge (hospice/Medicare/Joint Commission) Quality improvement and outcomes measurement Interdisciplinary team coordination Staff supervision and coaching Clinical policy and protocol development Case management and care coordination Electronic health record (EHR) systems Budgeting and resource allocation
