We are currently seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing visits in the Roscoe, IL area and surrounding portions of Boone and Winnebago Counties. Registered Nurse (RN) Responsibilities Manage and coordinate a patient caseload Complete Start of Care (SOC) OASIS assessments Perform ongoing skilled nursing visits and patient evaluations Collaborate with physicians and interdisciplinary healthcare team members Develop, implement, and update individualized care plans Educate patients, caregivers, and family members regarding disease management and treatment plans Monitor patient progress and communicate changes in condition to physicians and clinical leadership Document care accurately in Axxess EMR Provide wound care and other skilled nursing interventions as needed Ensure compliance with Medicare and agency standards of care Qualifications Current Illinois Registered Nurse (RN) license Minimum 1 year of Home Health experience (required) Minimum 1 year of OASIS experience (required) Wound care experience preferred Current CPR certification Valid driver's license and reliable transportation with auto insurance Strong clinical assessment, communication, and organizational skills Passion for providing compassionate, patient-centered care Join a Team That Values Experienced Nurses At Northwest Home Health & Rehab, we believe great patient outcomes start with experienced clinicians.
