Key Responsibilities:

Perform comprehensive home health risk assessments, each averaging 45 minutes per visit Evaluate patients medical histories, current conditions, and medication usage Educate patients regarding chronic illnesses, medication adherence, and preventive health Identify care gaps and provide actionable recommendations for resolution No treatment planning or prescription writing requiredfocus is on assessment and education

Qualifications:

Active Nurse Practitioner License in Illinois Board Certification via AANP or ANCC Graduate of an accredited MSN program Current CPR (BLS) certification At least six months of recent Nurse Practitioner experience preferred
