Job Description

Overview Join our compassionate healthcare team as a Hospice Nurse, where your expertise will provide comfort, dignity, and personalized care to patients facing life-limiting illnesses. In this vital role, you will deliver holistic nursing services in patients' homes, hospice facilities, or outpatient settings, ensuring their physical, emotional, and spiritual needs are met with kindness and professionalism. Y Responsibilities Conduct comprehensive patient assessments to determine care needs, including vital signs, symptom management, and physiological evaluations. Develop and implement individualized care plans aligned with patient goals and clinical best practices. Administer medications and treatments while monitoring for adverse reactions or changes in condition. Coordinate with interdisciplinary teams to facilitate discharge planning, case management, and post-acute care transitions. Provide emotional support and education to patients and families about disease progression, symptom control, and end-of-life options. Document all patient interactions accurately in medical records while adhering to HIPAA regulations. Engage in ongoing patient monitoring through diagnostic evaluation, intake assessments, and regular follow-ups to ensure comfort and quality of life. Experience Experience in specialized areas such as geriatrics, behavioral health, infectious disease care, or post-acute outpatient clinics is highly valued. Knowledge of medical terminology, physiology, utilization management, case management, discharge planning, and medical records is essential. Prior experience working with managed care organizations or in inpatient settings will enhance your ability to deliver comprehensive hospice services effectively. $20,000.00 - $40,000.00 per year
