  • Full-time, Contract Job Summary We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our dynamic healthcare team.
In this vital role, you will coordinate patient care across various settings, ensuring seamless transitions from inpatient to outpatient services, post-acute care, and community resources. Your expertise in clinical assessment, discharge planning, and utilization management will drive improved patient outcomes and optimize healthcare delivery. Responsibilities Conduct comprehensive patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine care needs. Develop individualized care plans by collaborating with multidisciplinary teams, including physicians, therapists, and social workers. Manage inpatient and outpatient cases across diverse specialties such as critical care, emergency medicine, pediatrics, geriatrics, and behavioral health. Coordinate discharge planning and post-acute care arrangements to ensure smooth transitions and continuity of care. Utilize medical records and diagnostic evaluation data to inform case management strategies while maintaining strict compliance with HIPAA regulations. Perform utilization management by reviewing hospital admissions, outpatient visits, and diagnostic procedures for appropriateness and efficiency. Engage in patient service activities such as intake assessments, patient education, and follow-up to promote health literacy and adherence. Qualifications Valid RN license with current registration; experience in hospital or clinical settings required. Critical care experience preferred, including ICU or Level I/II trauma center exposure. Strong background in medical-surgical nursing, emergency room experience, or managed care environments. Knowledge of physiology, medical terminology, diagnostic evaluation processes, and discharge planning protocols. Experience working with diverse populations such as pediatrics, geriatrics, or behavioral health is highly desirable. Excellent patient assessment skills combined with the ability to interpret complex clinical data. Familiarity with hospital records systems and outpatient clinic workflows; proficiency in HIPAA compliance standards.
