Provides and directs the provision of home nursing care based on agency policies and procedures, the competent application of the nursing parent. Complies with the documentation requirement of agency and regulatory agencies. Provide care to patients with acute and chronic wounds through the implementation of nursing procedures consistent with the medical plan of care and WOCN standards of practice in the management of patients with wounds. We are seeking a compassionate and experienced Home Health Skilled Nurse (Registered Nurse) to join our growing team. As a Visiting Nurse , you will provide one-on-one skilled nursing care to patients in their homes, helping them recover and maintain their health with dignity and comfort. Looking for coverage for the following IL counties: DeKalb, Will, Kankakee, Grundy counties Responsibilities

• Provide skilled nursing care in accordance with the patient's care plan
• Perform assessments, wound care, medication management, PICC line care, catheter care, and other skilled procedures
• Monitor patient health, progress, and communicate with physicians and care teams
• Educate patients and caregivers on disease management and treatment plans
• Maintain accurate and timely documentation using Axxess System
• Collaborate with interdisciplinary team for comprehensive care delivery
• Adhere to all agency, local, state, and federal regulations. Qualifications
• Current and valid RN in the state of Illinois
• Minimum of [1] year of clinical experience (home health experience preferred)
• CPR Certification (BLS)
• Valid driver's license, reliable transportation, and auto insurance
