Make a difference in community healthcare by delivering comprehensive in-home assessments as a Nurse Practitioner. This per diem position offers flexibility and autonomy, providing care for Medicare Advantage plan members across Ogle County. Youll perform annual and post-discharge health assessments for patients residing in areas such as Rochelle, Byron, and Oregon. Each evaluation is focused on risk assessment, with no requirements for prescribing or developing treatment plans.

Key Responsibilities:

Conduct thorough annual risk assessments in patients homes. Review and evaluate current and prior medical history, medical conditions, and medication use. Provide clear education and guidance to patients about their conditions and medication regimens. Identify any gaps in care or opportunities for improved health outcomes and recommend appropriate actions. Maintain organized and accurate documentation of each encounter.

Qualifications:

Unrestricted Nurse Practitioner License in Illinois. Current Basic Life Support (BLS/CPR) certification. Board certified by the AANP or ANCC. A minimum of 6 months experience practicing as a Nurse Practitioner is preferred. Master of Science in Nursing (MSN) from an accredited program.
