Signature HealthCARE seeks a compassionate Registered Nurse to deliver resident-centered care in our long-term and post-acute community. The RN will assess, plan, implement, and evaluate individualized care plans while ensuring safety, dignity, and clinical excellence. Responsibilities include medication administration, wound and IV care, coordination with an interdisciplinary team, and clear communication with residents and families. Responsibilities Assess, plan, implement, and evaluate individualized nursing care for assigned residents Administer medications and treatments, including wound and IV therapy per physician orders Monitor resident conditions, document accurately, and report changes promptly Collaborate with an interdisciplinary team to coordinate resident care and discharge planning Educate residents and families on diagnoses, treatments, and self-care strategies Supervise, support, and mentor CNAs and other support staff during shifts Maintain compliance with facility policies, state and federal regulations, and infection control standards Participate in quality improvement initiatives and performance metrics to enhance outcomes Use electronic health records (EHR) for accurate, timely documentation Support a resident-centered, faith-based, and mission-driven culture of compassion and dignity Required Skills Patient assessment Medication administration Wound care IV therapy and venipuncture Care planning and care coordination Electronic Health Records (EHR) documentation Infection prevention and control Geriatric and long-term care nursing Clinical documentation and reporting Patient and family education
