Singlepoint Healthcare is looking for an Intake Nurse Specialist to help coordinate safe, smooth, and compassionate home infusion care for patients with inflammatory diseases. What You'll Do ✔️ Assess patients for home infusion therapy ✔️ Coordinate care plans, supplies, and delivery schedules ✔️ Partner with home health agencies, referral sources, and pharmacy teams ✔️ Support patient and caregiver education ✔️ Monitor progress, therapy compliance, and care needs ✔️ Keep clinical documentation accurate and timely What You'll Bring ✅ RN or LPN license in the state of practice ✅ Associate's degree ✅ At least 1 year of nursing experience in home health or infusion therapy ✅ Knowledge of infusion techniques, home care protocols, and patient education ✅ Strong communication, organization, and patient-first mindset Why Singlepoint?
