Position Summary The MDS Coordinator plays a critical role in ensuring the accurate and timely completion of all Medicare and Medicaid case-mix documentation, supporting proper reimbursement and high-quality resident care. This position oversees the full MDS process, including assessments, care planning, interdisciplinary coordination, and regulatory compliance. Key Responsibilities Complete and oversee all MDS assessments (PPS, quarterly, annual, and significant change) Coordinate interdisciplinary team participation in MDS completion Ensure accuracy, timeliness, and regulatory compliance of all documentation Maintain MDS schedules and tracking for all residents Submit and verify MDS assessments electronically Lead and coordinate resident care conferences Collaborate with clinical teams to develop and update care plans Monitor outcomes and adjust care plans as needed Manage level-of-care changes and communicate updates to all departments Ensure proper submission of required documentation to state agencies Qualifications Current, unencumbered RN license Long-term care experience preferred Strong communication and organizational skills Ability to work collaboratively across departments Physical Requirements Ability to sit, stand, walk, and bend regularly Ability to lift and/or move up to 25 pounds Visual and auditory ability for effective communication Work environment with moderate noise levels Join a team that values your expertise and supports your growth.
