Key Responsibilities:

Conduct detailed home health risk assessments, typically lasting 45 minutes per patient Evaluate comprehensive health history, current and prior medical conditions, and medication usage Provide education and guidance regarding medical conditions and prescribed medications during each visit Identify care gaps and recommend appropriate next steps to ensure member health Focus exclusively on assessmentsno treatment planning or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Indiana Board certification through AANP or ANCC Current BLS (CPR) certification Master of Science in Nursing (MSN) from an accredited institution At least 6 months of relevant Nurse Practitioner experience is preferred Youll appreciate a straightforward per evaluation earning structure, with each assessment compensated at $80$90, plus a mileage stipend and administrative time allowances.
