Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Services shall include needed nursing and medical care as well as case management coordination and activities of daily living assistance to assure a comprehensive and holistic approach to meet individual needs and assist the consumer in obtaining and maintaining psychiatric stability.

Education:

RN; with Bachelor's degree in nursing or related health/mental health field preferred; Licensed Practice Nurse accepted.

Experience:

Minimum 2 years nursing experience; preferably in working with adults with long-term serious mental illness.

Licenses:

As required by degree. First shift,full time Monday - Friday, 8a-5p Salary based on experience. Excellent benefits EOE
