Job Description

MDS Coordinator (RN or LPN) Join our team at Transcendent Healthcare of Boonville! We're looking for an experienced MDS Coordinator (RN or LPN) to ensure accurate and timely resident assessments, compliance with regulations, and support for quality resident care.
What You'll Do:
Complete and submit MDS assessments per CMS and facility guidelines. Work closely with nursing, therapy, and interdisciplinary teams to ensure accurate coding and documentation. Monitor PDPM data for accuracy and compliance. Participate in care plan meetings and quality programs. Educate and support staff on documentation and MDS updates.
What We're Looking For:
Active RN or LPN license (Indiana) . Prior MDS and long-term care experience . Strong attention to detail, organization, and teamwork. Familiarity with MDS, PDPM, CMI, and RAI guidelines .
