Signature HealthCARE is seeking a compassionate Registered Nurse to deliver resident-centered care in our long-term care and post-acute rehab community. The RN will assess residents, develop and implement care plans, administer medications and treatments, and coordinate with an interdisciplinary team to support recovery, comfort, and dignity. Responsibilities include monitoring clinical status, documenting in the EHR, educating residents and families, and mentoring CNAs and LPNs. Responsibilities Assess residents' conditions, perform head-to-toe assessments, and monitor changes in status Develop, implement, and update individualized care plans in collaboration with the care team Administer medications and treatments safely and accurately per physician orders Provide wound care, IV therapy, and other skilled nursing interventions as permitted by scope and policy Document assessments, interventions, and outcomes in the Electronic Health Record (EHR) timely and accurately Coordinate care with physicians, therapists, social services, and other interdisciplinary team members Educate residents and families about diagnoses, medications, and care plans to support understanding and participation Supervise, support, and mentor CNAs and LPNs to ensure high standards of resident care Participate in quality improvement, infection control, and safety initiatives Uphold resident rights, dignity, and person-centered, faith-based values in all interactions Required Skills Patient assessment Care planning Medication administration Intravenous (IV) therapy Wound care management Electronic Health Record (EHR) documentation Infection prevention and control Geriatric and rehab nursing care Patient and family education Interdisciplinary team collaboration
