Key Responsibilities:

Perform comprehensive annual health risk assessments in home settings, averaging 45 minutes each Review and assess medical history, current conditions, and medications for Medicare Advantage members Deliver patient education regarding health conditions and medication management Identify care gaps, offering recommendations to promote patient health and close those gaps Maintain a focus on assessment and education; no need to create treatment plans or prescribe medications

Qualifications:
