Job Description
Signature HealthCARE seeks a compassionate Registered Nurse to deliver resident-centered care in our long-term and post-acute facilities. The RN will assess and monitor residents, administer medications and treatments, and collaborate with an interdisciplinary team to support optimal outcomes. Responsibilities include care planning, documenting clinical changes, educating residents and families, and promoting dignity and quality of life. Responsibilities Conduct resident assessments and monitor changes in condition Administer medications, treatments, and wound care per provider orders Develop, update, and implement individualized care plans Document care, assessments, and interventions accurately and timely in the EMRCollaborate with physicians, therapists, CNAs, and interdisciplinary team members Educate residents and families on care plans, medications, and health management Ensure compliance with clinical policies, safety standards, and regulatory requirements Support quality improvement initiatives and best practices in resident care Provide leadership, guidance, and support to CNAs and other support staff Promote a resident-centered, dignified, and compassionate care environment Required Skills Patient assessment Medication administration Wound care management Care planning and care coordination Electronic medical record (EMR) documentation Infection prevention and control IV therapy and vein access Vital signs monitoring and interpretation Discharge and transition planning Patient and family education
