Job Description

Signature HealthCARE seeks a compassionate Registered Nurse to deliver person-centered care in our long-term and post-acute facilities. The RN will assess residents, develop and implement care plans, administer medications and treatments, and coordinate with physicians, therapists, and CNAs to support optimal outcomes. You will monitor changes in condition, document accurately, educate residents and families, and promote dignity, safety, and comfort. In our resident-centered, faith-based, mission-driven culture, you'll collaborate with an interdisciplinary team, mentor staff, and participate in quality improvement while accessing ongoing training and growth opportunities. Responsibilities Assess residents' physical and psychosocial status and develop individualized care plans Administer medications and treatments per provider orders and facility protocols Monitor, document, and report changes in residents' conditions to providers and families Coordinate care with physicians, therapists, CNAs, and other interdisciplinary team members Perform wound care, IV therapy, and other skilled nursing procedures as permitted by licensure Educate residents and families on care plans, medications, and health management Ensure adherence to infection control, safety, and regulatory standards in the facility Supervise, support, and mentor CNAs and other assigned staff on the unit Participate in care conferences, rounding, and interdisciplinary planning Contribute to quality improvement, regulatory readiness, and best-practice initiatives Required Skills Patient assessment Care planning and care coordination Medication administration and reconciliation Electronic Health Record (EHR) documentation Wound care management IV therapy and venipuncture Infection prevention and control Geriatric and long-term care nursing Fall prevention and safety management Patient and family education
