Flexible per diem opportunity for an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members. This role involves visiting patients in Columbia City and throughout Whitley County, IN, including communities such as South Whitley and Churubusco. The primary responsibility will be conducting thorough, structured health assessments in a home setting, with each visit typically lasting about 45 minutes.

Key Responsibilities:

Complete annual health risk assessments in patient homes as part of Medicare Advantage initiatives Review and document past and current medical history, health conditions, and medication use Provide patient education regarding health conditions and medication management Identify gaps in care and offer recommendations to help improve patient outcomes No requirements for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in the state of Indiana Current CPR certification (BLS) Board Certified through AANP or ANCC Completion of a Master of Science in Nursing (MSN) from an accredited program At least 6 months of recent Nurse Practitioner experience preferred Compensation for this role is between $80-$90 per completed evaluation, with additional mileage stipend and payment for administrative time.
