Deliver comprehensive healthcare in the community with a rewarding per diem opportunity for an experienced Nurse Practitioner focused on home health risk assessments. In this role, you will visit Medicare Advantage plan members throughout various areas of Howard County, conducting thorough evaluations in the comfort of their own homes. Each assessment typically lasts around 45 minutes and may be an annual health review or a post-discharge follow-up.

Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review and evaluate current/past medical and medication history Deliver education and guidance related to medical conditions and medications Identify gaps in care and recommend solutions to help close those gaps No requirement to develop treatment plans or prescribe medications

Qualifications:

Valid Nurse Practitioner license in Indiana Board Certification through AANP or ANCC is required Current CPR certification (BLS) At least 6 months of recent NP experience preferred MSN degree from an accredited nursing program
