Job Description

Signature HealthCARE is seeking a compassionate Registered Nurse to join our Healthcare & Medical Services team. In this role, you will provide resident-centered nursing care, complete assessments, develop and implement care plans, administer medications, and collaborate with an interdisciplinary team to ensure optimal outcomes. You will educate patients and families, uphold regulatory standards, and support quality improvement initiatives. Responsibilities Provide direct, resident-centered nursing care, including assessments, medication administration, and treatments. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary care team. Monitor residents' conditions, recognize and respond to changes in status, and communicate critical findings to providers and families. Document all assessments, interventions, and outcomes accurately and promptly in the electronic health record system. Educate residents and families on diagnoses, medications, treatments, and self-care practices to support optimal health outcomes. Coordinate and oversee the work of certified nursing assistants and other support staff to ensure consistent, high-quality care. Adhere to infection control, safety, and regulatory standards while promoting a clean, safe, and dignified environment for residents. Participate in quality improvement initiatives, audits, and educational activities to support continuous improvement and evidence-based practice. Collaborate with physicians, therapists, social workers, and other team members to plan and deliver comprehensive, integrated care.
