Play a vital role in supporting patient care in a flexible, per diem home health setting as a Nurse Practitioner specializing in risk assessments. This position involves independent in-home visits to Medicare Advantage members throughout Shelby County, including communities such as Shelbyville, Fairland, and Morristown. Each home health risk assessment requires about 45 minutes and may include either annual health reviews or post-discharge visits.

Key Responsibilities:

Conduct comprehensive annual risk assessments for Medicare Advantage enrollees in their homes Review patients medical history, current conditions, and medication regimens Educate patients regarding their health conditions and prescribed medications Identify gaps in care, recommending actionable steps for improvement No responsibility for creating detailed treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Completed MSN from an accredited nursing school Current board certification through either the AANP or ANCC Possession of valid CPR (BLS) certification At least six months of experience as a Nurse Practitioner is preferred
