Schedule:

Full-time, Monday through Friday Potential for on-call hours Structured daily visit expectations

Key Responsibilities:

Deliver in-home skilled nursing care, including wound care, medication management, and disease monitoring Perform comprehensive assessments and complete all OASIS documentation accurately and on time Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams Provide patient and family education to support health goals and safety Communicate any patient status changes to the clinical team promptly

Qualifications:

Active RN license (unrestricted) At least 1 year of recent home health RN experience OASIS proficiency is required Experience with HomeCare HomeBase is strongly preferred Ability to manage a full-time schedule with daily patient visits Reliable transportation and valid drivers license
