Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage health plan members Review and document prior and current medical history, ongoing medical conditions, and medication regimens Educate patients on medical conditions and the importance of adhering to medication plans Identify care gaps and make recommendations for appropriate follow-up or interventions as needed No requirement to create formal treatment plans or prescribe medications

Qualifications:

Current and unrestricted Nurse Practitioner license in Indiana Valid BLS certification (CPR) Board certification from the AANP or ANCC Minimum of six months experience as a Nurse Practitioner is preferred Masters degree (MSN) from an accredited nursing program Evaluations average 45 minutes each, with visits varying in naturefrom annual health assessments to post-discharge check-ins. Pay ranges from $80-$90 per completed evaluation, plus a mileage stipend and administrative time.
