We are expanding our healthcare staffing team and seeking 5 qualified nurses for flexible agency assignments. Scheduling will be based on employee availability and the needs of our healthcare partners. Hours are not guaranteed and may vary by assignment. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews, ensuring accurate documentation within electronic health records (EHR) systems such as Epic or Cerner. Administer medications via IV infusion, injections, or other routes while adhering to aseptic techniques and safety protocols. Provide specialized wound care, including suturing and ostomy management, tailored to individual patient needs. Manage complex cases involving chronic conditions such as heart failure, diabetes, or neurological disorders; coordinate with multidisciplinary teams to optimize patient outcomes. Educate patients and their families on disease management, medication adherence, nutrition, and self-care strategies aligned with NCQA standards and best practices. Perform diagnostic evaluations like laboratory specimen collection or phlebotomy; interpret lab results to inform ongoing treatment plans. Assist with advanced procedures such as catheterization, ventilator management, or dialysis support when necessary. Ensure compliance with HIPAA regulations by maintaining patient confidentiality and accurate medical documentation throughout all interactions. Triage urgent patient needs via telehealth or in-person visits; escalate critical cases promptly following trauma medicine or emergency response procedures. Collaborate with physicians during hospital transitions or discharge planning to facilitate seamless continuity of care across settings like skilled nursing facilities or assisted living communities. Qualifications Valid Registered Nurse (RN) license in good standing with current state certification. Proven experience in home health nursing with familiarity in medical-surgical care, geriatrics, or long-term care environments. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks; strong medical documentation skills. Expertise in managing complex patient needs including ICU experience, cardiac catheterization procedures, infectious disease care, or trauma medicine is highly desirable. Knowledge of ICD coding (ICD-10/ICD-9), CPT coding for procedures and diagnoses; familiarity with laboratory testing processes. Strong clinical skills including IV insertion, medication administration, aseptic technique and wound suturing. Ability to perform comprehensive physical examinations and vital signs assessment accurately. Experience working with individuals with disabilities or developmental delays; knowledge of behavioral health and memory care is advantageous. Excellent communication skills for effective patient education and interdisciplinary collaboration; triage experience preferred. Certifications such as Basic Life Support (BLS), Neonatal Resuscitation Program (NRP), or Critical Care certifications are a plus.
