Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members in patient homes Evaluate patients prior and current medical history, medical conditions, and medication usage Provide valuable education to patients regarding their health issues and medications Identify care gaps and share recommendations; no development of treatment plans or prescription duties required

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR (BLS) certification Board Certification through AANP or ANCC Minimum of 6 months current Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program Experience a compensation structure rewarding your expertiseearn between $80 and $90 per completed evaluation, plus benefit from a mileage stipend and paid administrative time.
