A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description

Job Summary This position coordinates care between patients, families, physicians, home health agencies, and other care providers so that our customers will receive high-quality cost-effective health and hospital services. Performs other duties as assigned. Minimum Job Requirements Bachelor Degree in Nursing from an accredited university -OR- Associate's Degree in Nursing from an accredited university and 2 years' Case Management experience. Current Indiana Registered Nurse License. Three (3) years of recent nursing experience in an acute hospital or related setting. BLS certification or must obtain during Orientation and prior to reporting to department. Preferred Job Requirements Certification in Case Management is preferred. Certification can be either Commission for Case Management Certification (CCMC) or American Case Management Association (ACMA) Skills / Knowledge / Abilities Ability to read, write, and follow directions. Excellent communication skills to effectively and tactfully communicate with patients and their families and guests, co-workers, managers, providers and other hospital personnel on a one-to-one basis using appropriate grammar, vocabulary and word usage. Interpersonal skills necessary in order to carry out duties in a professional manner, as well as the ability to work well under stress. Highly developed computer skills to include proficiency with Microsoft Office and the ability to learn and proficiently perform computer applications related to department operations and job function. Strong customer service skills. Analytical and statistical skills to comprehend and analyze healthcare data and reports. Strong organizational skills in order to manage multiple issues simultaneously combined with the ability to effectively prioritize work flow. Ability to be discreet and protect the integrity of any confidential matter or information encountered during the performance of job duties. Working Conditions Mostly works in typical office environment. Occasionally visits clinical environment with risk of exposure to blood, body fluid or other potentially infectious materials. May be at risk for electrical hazards or hazardous materials. Works with protected health information. Ability to work extended hours, holidays or weekends based on the needs of the department. Physical and Mental Activities, Tools and Equipment Use of computer and other standard office equipment. Extreme concentration and attention to detail. Lifts standard office supplies and equipment.
