Home - Fortis Job Summary Select Home Health is seeking skilled and compassionate PRN Registered Nurses to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and overall well-being.
Duties/Responsibilities:
Perform patient assessments and ongoing evaluations. Implement physician-ordered plans of care and complete required documentation (OASIS). Provide in-home nursing care for multiple patients per day. Educate patients and caregivers on medications, disease processes, and in-home care. Coordinate patient services and supervise LPNs/HHAs as needed. Communicate changes in patient condition to physicians and care teams. Maintain compliance with regulations and agency policies. Physical Requirements Ability to sit, stand, and drive for extended periods while providing in-home care. Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation. Regular use of a tablet for documentation, communication, and training.
