Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage members throughout Delaware County, including Muncie, Yorktown, and Chesterfield. Schedule:

Responsibilities:

Conduct comprehensive annual risk assessments in patient homes for a Medicare Advantage health plan Review and evaluate prior and current medical history, medical conditions, and medication regimens Educate patients on their medical conditions and prescribed medications Identify gaps in care and provide expert recommendations to address them

Qualifications:

Active Nurse Practitioner license in Indiana (IN) Board Certification from AANP or ANCC Current BLS/CPR certification MSN degree from an accredited nursing program At least 6 months recent experience as a Nurse Practitioner preferred
