Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes Review and evaluate members current and prior medical history, ongoing conditions, and medication profiles Deliver focused education to patients on medical conditions and proper medication use Identify gaps in care and offer actionable recommendations to ensure optimal patient outcomes Visits typically last approximately 45 minutes; duties do not include prescribing medications or creating treatment plans

Qualifications:

Active Nurse Practitioner license in Indiana Board Certification (AANP or ANCC) Current CPR (BLS) certification Master of Science in Nursing (MSN) from an accredited institution At least 6 months of recent nurse practitioner experience preferred This role offers highly competitive compensation, including $80$90 for each evaluation, plus a mileage stipend and compensation for administrative time.
