Responsibilities:

Complete thorough annual or post-discharge risk assessments in patients homes Review prior and current medical histories, underlying conditions, and medications Offer patient education regarding ongoing health conditions and treatments Identify care gaps, making targeted recommendations for follow-up Document all assessments and findings accurately Travel within Knox County to varying locations for each assignment

Qualifications:

Active Nurse Practitioner license in Indiana MSN degree from an accredited nursing program Current Board Certification through AANP or ANCC Valid CPR certification (BLS required) At least 6 months experience as an NP preferred Outstanding patient communication and time management skills
