Signature HealthCARE seeks a compassionate Registered Nurse to deliver resident-centered clinical care in our long-term and post-acute facility. The RN will assess residents, administer medications and treatments, coordinate care plans, and collaborate with an interdisciplinary team to promote dignity, safety, and recovery. Responsibilities Assess residents' conditions and develop, implement, and update individualized care plans Administer medications, treatments, and therapies according to physician orders and facility protocols Monitor resident status, document accurately in EHR, and communicate changes to providers and families Collaborate with interdisciplinary team members to coordinate resident-centered, goal-oriented care Provide education and emotional support to residents and their families on conditions, treatments, and self-care Ensure compliance with infection control, safety, and regulatory standards in daily practice Supervise and support LPNs, CNAs, and other staff, promoting teamwork and professional conduct Participate in quality improvement initiatives and contribute to best-practice care processes Respond to emergencies, perform required clinical interventions, and escalate concerns appropriately Support a faith-based, mission-driven culture focused on compassion, dignity, and continuous improvement Required Skills Patient assessment and clinical judgment Medication administration and IV therapy Care planning and care coordination Wound care and skin integrity management Geriatric and long-term care nursing Electronic Health Record (EHR) documentation Infection prevention and control Vital signs monitoring and early deterioration recognition Patient and family education Team leadership and delegation to support staff
