Flexible shifts available Monday through Sunday Create your own weekly schedule Commit to a minimum number of hours each week or every other week

Key Responsibilities:

Complete home health risk assessments, each taking approximately 45 minutes, at patients residences Gather and evaluate prior and current medical history, medical conditions, and medication history Educate members about their health conditions and medication management Identify care gaps, providing actionable recommendations for follow-up Focus exclusively on assessmentsno treatment plan creation or prescribing required

Qualifications:

Active Nurse Practitioner License in IN Current Basic Life Support (BLS/CPR) certification Board certification through AANP or ANCC Six months or more of current Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution
