As a Nurse Practitioner specializing in home health risk assessments, youll make a significant impact by delivering essential services to Medicare Advantage plan members throughout Parke County, IN. In this per diem opportunity, youll visit members in their homeswhether in Rockville, Montezuma, Marshall, or other areas within Adams Township and Parke Countyconducting thorough evaluations and offering valuable support right where patients need it most.

Key Responsibilities:

Complete home health risk assessments for Medicare Advantage members, typically within a 45-minute visit Review each members medical history, current conditions, and medication profile Educate members about their diagnoses, conditions, and prescribed medications Identify gaps in care and offer actionable recommendations to close these gaps Visits may include annual health checkups or post-discharge assessments No requirements to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Indiana MSN from an accredited nursing school Current BLS (CPR) certification Board Certified through AANP or ANCC At least 6 months of recent NP experience preferred This role offers a compensation of $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
